Fear Factory was a fear level that was intended to be included in the final game. However, it was cut for not having any proper connection to the plot and for the minigame not being fun enough according to the developers. 

Location

The factory where the level was meant to take place can still be found outside of the map, by using the Unreal Engine Missing Script, cheat engines or other methods of hacking. It is located on the lakeside and has pipes sticking into it. It also has a large red and white striped pipe sticking out of its roof.

Gameplay

The player would have started in an empty and desolate factory, with only a basketball in front of them, then, mannequins will surround Nicky, forcing you to throw the ball, causing them to run after it. Repeat this a few times until you escape, this would have given you the super-throw, but that became the default ability.
